Need Help Registering?

ATTEND ONE OF OUR PATIENT REGISTRY FAIRS

Patient outreach and education will be posted as the events are booked. If you would like to see a patient registry fair in your area, please call Jennifer White 410-487-8071 and we will do our best to accommodate you request.

If you plan to attend one of the Patient Registry Fairs, please RSVP by sending an email to Mdh.infoandregistration@maryland.gov.  On your RSVP please indicate the date and location you plan to attend and the number of patients or caregivers registering.

What to bring to the Registration Fair

If you are registering yourself as a patient or caregiver bring:

  • You will need to bring two documentsd documents that have your current address as a resident of Maryland.
  • Your government ID (driver’s license, MVA card, passport or military ID)

NOTE: If your government ID does not show your current address, you must also bring TWO document that proves your current address, such as a utility bill, bank statement, rental contract, mortgage statement, or a federal or state document with your name and current address.

If you are registering an adult who will not be present at the registration fair, bring:

  • A passport photo of the applicant. If you cannot get a passport photo taken, you can bring another photo, but please make sure it meets our photo guidelines.
  • A copy of the applicant’s government ID (driver’s license, MVA card, passport or military ID)


NOTE: If the applicant’s government ID does not show his/her current address, you must also bring TWO document that proves his/her current address, such as a utility bill, bank statement, rental contract, mortgage statement, or a federal or state document with his/her name and current address.

If you are registering a minor patient:

  • A passport photo of the minor. If you cannot get a passport photo taken, you can bring another photo, but please make sure it meets our photo guidelines. We can take a photo of the minor, if he/she is present at the registration fair.
  • A notarized copy of the MMCC Minor Patient Form.
  • Birth Certificate, Adoption Papers, or Court Order that establishes you to be the parent or legal guardian of the minor.
  • Your government ID (driver’s license, MVA card, passport or military ID).
